Enrollment

Before your child can have his/her first day at Agape, there are several items we must have:

• Proof of physical examination within one year of enrollment
• Completed, updated Washington State CERTIFICATE OF IMMUNIZATION
• Completed enrollment forms
• One month’s tuition in advance
• Emergency card form
• A written list of foods which the child cannot consume because of allergies with an individual plan filled out (if applicable)
• A signed consent form for child to go on neighborhood outings and field trips
• A signed agreement for the child care provider to provide transportation in case of any  emergency
• A written consent for the child to receive emergency medical care.

Agape accepts children whose care may be subsidized by the Washington State Department of Social and Health Services, if space allows.
